What are the credential requirements acceptable by CLIA to be a Clinical Laboratory Director in Public Health?

What do you mean by Director in Public Health, do you mean public health microbiology? In general you need to have a PhD degree in the field you want to direct (clinical chemistry, toxicology, molecular genetic, cytogenetic, microbiology) and have appropriate post-doc training in your field before hospital/lab will let you direct that section.
For clinical chemistry the most common board is the American Board of Clinical Chemistry http://apps.aacc.org/abcc/

For toxicology it is the American Board of Toxicology http://www.abtox.org/
For genetic, cytogenetic, it is the American Board of Medical Genetic http://www.abmg.org/

For immunology it is the American Board of Medical Laboratory Immunology
