As a shop steward and safety rep I was approached by one of my members about a task he was asked to carry out that he considered to be unsafe,after looking at the task I agreed with him and informed the management of my investigation and my conclusion.After a disagreement on the issue I said that if they supplied me with a signed statement saying that the task was safe then the member would attempt it.Of course they would never have done that so an alternative method was adopted requiring an extra person.

I assume you are having trouble with your homework and expect others to do it for you!
I had a chap working on a capstan lathe in a machine-shop and he made a mistake while setting this machine (he left an Allen key in the capstan and forgot it was there). When he realised his mistake he knew this would cause damage to the machine so he tried to grab the key as the capstan indexed around, which sure enough nicely crunched 3 fingers on his hand. We had safety inspectors investigate and at the end of the day for that type of machine there was no practical mechanical solution. The only action we could take was to retrain the man. Stupidity in the workplace cannot always be prevented.

The fire alarm went off. I immediately called the fire department and evacuated the building.
