it reallly doesnt happen! if you get working tax credit then you should be entitled to a percentage of your childcare fees but its up to the inland revenue not your employer, and it depends on your income.

Your chances of winning the lottery jackpot are higher than getting paid childcare! (Seriously)

In the United States it's not common. You might get free child care if you are a preschool teacher and your children attend where you work. Some larger corporations offer a discount at certain child care centers to their employees, some contract with certain child care centers to provide back up child care for their employees. It's not likely to find one that will pay your entire child care expense, unless you are really, really good at what you do and the only thing they can offer you as a perk that's more than what the other company will offer is child care costs. Then you might have some pull. I'm a preschool teacher and as a part of job offer packages I have been offered free child care, 100% paid benefits, extra earned time, vacation days, etc. That is our field though, they can offer that to help hook a really good teacher. Our field isn't the highest paid, so they use perks to attract and keep staff.

Not common at all, in the 20 years I've been working I don't know one person who gets that as a benefit.

It is not common but depending where you are it might be possible. Where I live (Canada) the government has installed a new tax break for companies who will open a daycare in their office. So yes it might be interesting for some companies to supply free or very affordable daycare to their employees.
