You would have to prove any wrong doing, the people that investigate will not just take your word for it,especially if you dont give your name.

Go to your state government's website. Look for where to send complaints. You CAN remain completely anonymous. Just provide as many details as possible about the issues because they won't be able to contact you if they have questions and you remain anonymous. Good for you for reporting them though--there are way too many bad nursing homes.

If you don't live in that Nursing Home or none of your relatives lives in the place you are going to report, you can give your name. During investigation takes place, some Nursing Home may retaliate against the elderly especially when they have lost their ability to communicate. You can report the Nursing Home to your State Legislator office, under Department of Children & Family, the Adult Services or the Elder Care Office by writing to them or by calling them.

You should definitely report a nursing home if you suspect they are not giving quality care to residents or if there are other potential problems.
The links at the bottom of this page: http://nursinghomelawyer.com/nursing_home_law_firm/nursing_home_research/nursing_home_abuse/index.htm
should help you with the reporting process.
Good luck, and you're doing the right thing.

You can make a complaint to the Royal Care Commission and they will investigate by sending one of thier inspectors on a spot check. It could be any time of night or day when the nursing home is least expecting it. The Care Commision are trying to get all homes and day centres to evaluate themselves between 1-6 So put everything down on paper and write or phone your concerns you dont have to give your name. Think they are also online

You don't say which Country.
In the UK, Nursing Homes are Registered by the local Health Authority; the Duty Social Worker at Social Services will be able to give you contact details, or you could ask at Citizens Advice Bureau.
Complaints will be investigated whether you give your name or not.
