I have a job interview for a Operations Manager through a recruiter but she mislead them about something?

I talked to the recruiter and we never went over why I lost my last job (just when I lost it). I asked the recruiter to send me the resume that she sent to the client (since I wanted to make sure I brought the same exact one). The form she sent me said that I was laid off from my last job because the program was completed. That is not true. One of my employees made a mistake and being the supervisor took responsibility for it. I have always been honest about it (and had several phone interviews end quickly after being honest) because I don't want some one to check references and to find out something different.
The recruiter got my information from another recruiter that I never actually spoke with either. How do I address this in the interview?

You just tell them that the reason you lost your job was one of your subordinates messed up and you owned up to it.

First of all - you should know, unless it was illegal wrongdoings - they will not know WHY you got fired. Your former employee can not talk about negative things about you.
Secondly, you should be proud that you are so honest - but sell yourself in a positive way. Don't bring up the negatives!

I think firefightin mamma is right that they are not allowed by law to speak nagatively about someone. I'll tell you a secret. I have had only 1 job actually call for my refs and same job is only job Ive had that has not given a drug test while others have. My point is that I really dont think they do very often call for refs.
