I am going on a cruise and trying to find forms for medical power of attorney?

We are going on a cruise and leaving the kids at home with a sitter. My friend suggested that we make out some forms for medical power of attorney for the kids, incase anything happens to them and they need medical attention while we are gone. Where do we find these forms?

You can find them online and should also be able to find (online) your state's requirements or imitations. Also check with your local office for the aging. They usually have them because they have seminars on POAs, living wills, etc. Your doctor or hospital or school system may also have them. Your situation is not unusual.
