What questions would they ask me this time? Is it tougher? any medical secretary here pleassee??

You're in the closer selections already. Try to be releaxed and natural. You wouldn't have come so far if you weren't so good already. Now they might want to see how you'd do when more critical problems arise and complex solving skills and experience might come in handy. If you don't make it, it doesn't mean that you're not good, just that you may not have more experience than someone else that anwered the questions or scored in a more experienced way. my daughters second med sec interview was actually one and half hours of tests, like typing speed, MD Word knowledge, etc.
