Get rid of the date of birth, add your name and contact info to the top your summary needs to be specific to the company you are applying to. If it is an ad, put in the position and company name and that you will complete what ever they are asking for. Then goes your education, lose the middle school, I would lose the academic achievements, change Athletic Achievements to memberships and list sport and academic teams you belong to, mark if you hold an office, like captain or president and lose that paragraph at the bottom. You are still in school, short resumes are expected, and no one wants to read through the rest of that crap!
