| Coverage Choices |

| Coverage Choices provide flexibility in the type and cost of your employee benefits. The program has a wide range of benefit options. You have choices in medical, dental, vision, life and accidental death & dismemberment insurance, additional employee and dependent life insurance, long term disability, among other benefits. The Company gives you a budget of Cypress Dollars bi-weekly to pay for your Coverage Choices. Once you make all your choices, if the premium cost ends up less than your benefit budget (1) you can take the difference as taxable compensation added to your regular pay, or (2) you can apply the difference to your flexible spending account (for health care or dependent care) on a pre-tax basis. If you choose coverage with a total bi-weekly premium cost of more than your Cypress Dollars, the additional costs will be deducted from your regular pay on a pre-tax basis (for medical, dental and vision) or post-tax basis (for life, supplemental life, AD&D and LTD). |
