Programs EPSDT—A service informing Iowa Title 19 recipients of available child health services and care coordination. Child Health & WIC—A program which provides health screenings for children from birth to 21 years. Coordinates with Women, Infant and Children(WIC) nutrition services. Vaccines For Children (VFC)—A service providing vaccines at no cost to children birth to 18 years who are not covered by insurance. HIV/AIDS—A program that provides primary health care to individuals living with HIV/AIDS as well as offering early intervention through HIV testing and outreach. Click here for more details. DHS—An Income Maintenance Worker for Iowa is now located at the Health Center to assist with Title 19 and HAWK-i applications. Interpretive Services Spanish and Vietnamese interpreters are staffed full time. French interpreters are available upon request. Services for the hearing impaired are also available when needed. Pharmacy The pharmacy offers prescription medication at a discounted price to our patients. The pharmacist works closely with providers, staff and patients in providing education and consultative services. | | Dental Service Dental services are available for you and your family by board certified dentists practicing in the area of general dentistry. Please take the time to find out about our I-Smiles program for children 12 years of age and younger! Emergency services are also provided on a walk-in basis at time permits. Referral services are available for difficult cases and situations that are beyond the scope of practice for a general dentist. In Case of Emergency For an emergency during office hours, please call the office and state that you have an emergency. The nurse or physician will speak with you immediately and give you further instructions. If an emergency occurs after office hours, you may call the operator at St. Luke's Regional Medical Center, 712-279-3500. The provider will return your call quickly. If you need to be seen, the provider will direct you to go to the Emergency Room. Social Services The Social Services Department is available to provide case management services, assist in referrals and financial arrangements, and short-term counseling. Outreach activities promote HAWK-i enrollment and patient education/prevention programs. Financial Services Account Representatives are available to assist Health Center's patients. They help you complete forms, access assistance for payment of services, determine if you are eligible for financial discounts and schedule appointments. The Health Center files insurance claims of your behalf. You are to bring your insurance card, Title 19 and/or Medicare card with you to each appointment. A sliding fee scale is available for patients with no insurance or Title 19. Patients interested in applying for this discount need to bring financial information with them to their first appointment. |
