About Grace Healthcare
Grace Healthcare was founded in 1997 to manage Nursing Care and Rehabilitation Facilities developed or acquired by Sunland Corporation. Grace Healthcare's corporate offices are located in Chattanooga, Tennessee, and the company manages twenty two (22) facilities in nine states: Alabama, Arkansas, Colorado, Florida, Maryland, Michigan, Tennessee, Texas, and Virginia. Grace Healthcare also participates in the Federal Office of Inspector General's Corporate Compliance Program, which is a federally approved plan, whereby the member healthcare companies set up their own internal corporate program, sanctioned by the federal government, to promote and uphold recognized ethical and legal standards in the conduct of business with the federal government and the general public. Grace Healthcare Services
Grace Healthcare is a provider of a full range of health and residential services for the elderly or medically challenged resident.
•Rehabilitation Services: Grace Healthcare provides comprehensive Physical, Occupational and Speech therapy programs. The attending physician and licensed therapist develop individualized resident treatment plans using state of the art therapy techniques. Our therapy treatment programs include strengthening and reconditioning of muscles to improve coordination and endurance, treatments for pain management, post surgical treatments, prosthetic training, retraining in activities of daily living, home living skills, splinting, post stroke care including retraining swallowing, speech and communications skills.

•Skilled Nursing Services: These services are often provided after a hospital stay of at least 3 days and are usually covered by Medicare part A, if the resident is otherwise qualified for this program. An interdisciplinary care team consisting of the attending physician, registered nurse, social worker, activity professional, dietician or nutritionist, along with input from the resident and or family, come together to develop an individualized resident medical plan of care. This care plan is assessed frequently and changes made to help each resident achieve their optimum level of care; physically, socially and emotionally.
Other skilled services provided may include post surgical care, stroke recovery, IV therapy services, respiratory therapy, oxygen, and other medical gas administration, wound management, tracheostomy or illeostomy care, and tube feeding services. Some residents after completing these shorter term, high level treatment programs, may return home, or to a less intensive healthcare setting.
•Intermediate Care Services: These services are provided to those who require daily monitoring and management of a condition that cannot be provided in a home setting. These services include medication management, licensed nurse supervision, assistance with bathing, dressing, and eating and catheter or ostomy care. Residents under this level of care also benefit from the individualized, professional care planning process. Daily nursing restorative care may also include incontinence care, bowel and bladder training, and ambulation training and assistance.
•Respite Care: This service is a short term care program designed to give the family members a needed break from the extreme demands of caring for the chronically ill or handicapped person at home. If only for a week or a few weeks, respite care residents receive necessary services in a secure medical environment.
•Alzheimer’s Care: Many of our nursing centers have dedicated units for the treatment of Alzheimer’s disease and related disorders. These particular units are secure, safe environments especially focused to treat cognitive disorders. Special resident activities and programs are offered by employees who are trained in the care of those who are cognitively impaired.
•Grace Healthcare Nursing Centers: All Grace Healthcare Nursing Centers are licensed as Skilled Nursing Facilities under the particular state licensure of the state in which they are located. These centers are all organized with a licensed Administrator and Director of Nursing.
An interdisciplinary care team consisting of the attending physician, licensed nurses, social worker, activity professional, dietician or nutritionist, along with input from the resident and or family, come together to develop an individualized resident medical care plan. This plan of care is assessed frequently and changes made to help each resident achieve their optimum level of care; physically, socially and emotionally.
Grace Healthcare nursing centers accept Medicare, private insurance and private pay reimbursement, and most also accept Medicaid program reimbursement.
•Assisted Living Services: Some Grace Healthcare facilities offer this level of service for those residents needing less intensive services, yet need a safe, supportive residential environment. State licensed assisted living services include: housekeeping services, three meals daily, with snacks provided, laundry services, general monitoring for health and well being, assistance with medication management, assistance with dressing, bathing and eating, social activities, assistance with scheduling medical appointments, and 24 hour supervision by trained healthcare providers.
•Adult Day Care Services: Some of Grace Healthcare facilities provide adult day care services which offer a protective environment to those who may be living with family members, but cannot stay home alone during the day.
These services include: two meals and an afternoon snack, with special diets accommodated, social activities, beauty/barber shop services, supervision and basic health monitoring, and emergency nursing care. Services are generally provided from 7:00 A.M until 6:00 P.M. Monday through Friday. These services are offered currently at the Leewood Healthcare Center location in Annandale, VA.
•Independent Living Centers: Live life without the concerns of home ownership, yard work, or equipment maintenance. Reassert your independence and experience a special peace of mind, socialize with friends, live in a safe, secure environment with 24 hour emergency nursing response available. Grace Healthcare offers an independent life style with special amenities such as: social and recreational programs, transportation services, housekeeping services, security monitoring, restaurant style dining, postal services, all on a campus including assisted living services and skilled nursing services. These services are offered currently at the Bella Vista Arkansas location at the Concordia Healthcare Facility location in Bella Vista, AR.
