Job Details:
The ED/Admissions RN Case Manager is responsible to assess all ED and unplanned patient admissions during work hours. Assessment parameters to include identifying medical necessity criteria, corresponding correct registration status, and discussing findings with MD/IP requesting patient admission in order to facilitate appropriate patient placement. The ED/Admission CM is also responsible for directing an organized, patient-centered interdisciplinary team system that coordinates the delivery of healthcare services by continually monitoring, evaluating, and modifying the treatment plan to achieve optimal patient outcomes and efficacious use of resources. The NCM coordinates with physicians, Case Management Social Worker, and other health team members to expedite medically appropriate, cost-effective care and promote quality through evidence based medical practice throughout the health care system. Knowledge of current case management principles, third party payers requirements and discharge planning process. Knowledge of current nursing principles, management principles, as well as hospital organizational operations. Must be able to work calmly in stressful situations and be self-directed. Excellent organizational skills, as well as excellent and effective oral and written communications skills. Ability to adapt to constantly changing demands of outside payer groups. Demonstrates diplomacy, ability to collaborate with others and coordinate activities. Demonstrates good judgment and high degree of concentration. Ability to interpret information from medical records and make decisions. Knowledge of clinical databases and strong assessment skills.
Current licensure in the State of Montana as a Registered Nurse. Current Case Manager Certification is preferred. Current CPR certification, or completion of course within 3 months. Previous experience as a Case Manager; or 3 years current clinical hospital experience; or Nursing Management experience.
Position will rotate between day & night shifts.
