Providence Hood River Memorial Hospital has a current need for a part-time licensed Social Worker that provides compassionate service to patients at the Ray Yasui Dialysis Center for 24 hours per week.

General Summary: Provides services to patients and their families to support and maintain social functioning and adjustment to illness. Is responsible for conducting psychosocial evaluations, participating in team review of Patient progress and recommending changes in treatment based on the patient's current psychosocial needs.
Required: Must be an Oregon Licensed Clinical Social Worker (LCSW).Master of Social Work (M.S.W.).
Preferred: Bilingual, flexible with daytime hours, thorough understanding of Medicare part D and 2010 CMS guidelines, dialysis and medical social worker experience.
