Per Diem LPN: Girling Home Health Care

Girling Home Health is looking for a Per Diem Licensed Practical Nurse in Tulsa, OK and Oklahoma City, OK.

Provides care in accordance with physician orders, the patient's condition, and within accepted standards of practice, Agency policies, procedures and processes. Identifies changes in the patient's condition, advises patient care team, and implements appropriate interventions. Utilizes the individualized teaching plan to include education of patient's medical condition. Updates such plan as indicated. Adheres to nursing care and discharge plans. Provides on-call duties as required.

Qualifications:

Free from Medicare Sanctions (required)

Current LPN license (required)

Current CPR certification (required)

Adequate means of transportation to facilitate timely visits (required)

Minimum of two years experience as an LPN (preferred)

One year Home Health experience (preferred)
