Home Coordinator - FT

Experience working with individuals with developmental disabilities in community settings, experience working with challenging behaviors, and prior supervisory experience

Full-time position working 40 hours a week -1:00 PM to 10:00 PM Monday, Wednesday, and Friday and12:00 midnight to 6:00 PM Tuesday and Thursday.

Duties include, but not limited to coordinating daily client cares in our Supportive Living Program that serves individuals with developmental disabilities and traumatic brain injuries in their own home or apartment; train new staff in two households and provide on-going coaching on how to best support the clients; be available to clients and Community Support Specialists for guidance, support, and problem solving; assist in scheduling staff to fill existing openings and vacations; provide assistance with activities of daily living, such as money management, personal cares, cooking and cleaning; monitor household supplies and grocery shop for the homes; review daily paperwork completed by staff and assist with implementing changes needed; assist clients in planning and accessing a wide range of community activities and develop a weekly recreation calendar; assist client in reaching her/her goals; attend all team meetings to discuss progress/concerns; communicate with program manager weekly regarding the status of the clients; monitor medication procedures and check for medication errors, and offer suggestions on improving services to the clients.

Required Skills: ~outstanding organizational skills a must
~excellent verbal and written communication
~exceptional time management skills
~CNA/nursing background a plus
~work effectively as a member of the multi-disciplinary team
~comply with all agency, county, and state paperwork, annual reviews, and
other requirements
~knowledge of dual diagnosis a plus

Comments: This position requires the use of a personal car to travel to various places around Dane County. Mileage is reimbursed at a current rate of 0.485 per mile.
