Position Summary:
The Care Manager will support Evercare's Advanced Illness program by providing a holistic approach to case management, including addressing physical, emotional, spiritual and psycho-social needs of members and their families. The Care Manager will assure that members and families meeting the criteria for Advanced Illness have current, anticipatory and advanced care plans in place. By working effectively with the healthcare provider team, engaging with PCP as required, this role will also assure that members and families have goals of care respected and honored. In this position, you will provide expertise in pain and symptom management and have the opportunity to provide education and support to members/ families and all stakeholders. You will also be responsible for developing effective working relationships with our business' senior leadership at the site level and will work effectively across sister segments both internally and across the Enterprise. The Care Manager must completes required documentation in timely and accurate manner and will develops improved work flow as required. The successful candidate in this position will establish oneself as an expert in Advanced Illness and serve as a leader and mentor of others. This is a field-based role which requires local travel within your assigned geographical territory; assessments and follow-up visits will be completed in face-to-face patient visits. When not working in the field, you will work from a home-based office.
Qualifications
Qualifications:
- Current and unrestricted RN licensure in TX
- Undergraduate degree or equivalent experience required; BSN preferred
- Ideal clinical experience includes palliative care, hospice, home health, or community case management
- Successful candidates will be highly organized and possess excellent communication skills
