NP II-Population Mgt (RP20-Variable)

BASIC JOB PURPOSE
Under indirect supervision and as credentialed and privileged, MANAGES chronic disease or other special populations across continuum; COLLABORATES with staff to implement quality initiatives to optimize patient outcomes; PROVIDES consultation to other health care providers; PROVIDES services that are within scope of advanced nursing specialty practice aligned with graduate educational preparation; Independently PROVIDES theory and evidence-based care to promote optimal patient outcomes; ASSESSES patient health status and ESTABLISHES diagnosis/diagnoses; DEVELOPS, IMPLEMENTS and EVALUATES treatment plan; ORDERS, PERFORMS and/or INTERPRETS screening and diagnostic tests; PERFORMS therapeutic procedures or interventions; PROVIDES a variety of educational services; INITIATES consults and referrals; SERVES as care manager for patients with complex needs; PLANS, GUIDES, EVALUATES and DIRECTS nursing care provided by others; SERVES as role model, consultant and resource for advanced nursing knowledge and skills; PROVIDES clinical expertise to affect system-wide changes in organizations to improve programs of care; SUPPORTS compliance and Principles of Responsibility; MAINTAINS confidentiality; PROTECTS organizational assets; EXHIBITS ethics and integrity; ADHERES to applicable federal and state laws and regulations, accreditation and licensing requirements, policies and procedures; REPORTS and/or RESOLVES issues of non-compliance.
On regular, sustained basis, COOPERATES with other staff members both within and outside department in accomplishment of own job duties as well as assisting others in accomplishing theirs. SERVES as team player and role model for other employees in Organization; always exhibiting traits of courtesy, caring, helpfulness and respect. CONDUCTS self in a service-oriented manner that is attentive, pleasant, cooperative, sensitive, respectful and kind when dealing with members, visitors, public and all employees.
MINIMUM QUALIFICATIONS
EDUCATION/TRAINING
Masters degree in Nursing with specialty in adult, medical-surgical, pediatric, gerontologic, acute or family nursing or womens health.
Equivalent combination of education & experience may be considered in lieu of requirements.
LICENSURE/CERTIFICATION
Valid Hawaii RN License (must meet education requirement(s) for Hawaii State licensure).
Valid Hawaii APRN Recognition (must meet education requirement(s) for Hawaii State recognition).
Certified Nurse Practitioner (NP) in adult, family, medical, womens health, pediatric, gerontologic, acute care or family nurse practitioner as accepted by the Hawaii State Board of Nursing.
Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification.
Valid drivers license.
EXPERIENCE/OTHER
Three months acute care training/experience.
Demonstrated knowledge of and skill in the following: customer service, group process facilitation, conflict resolution, counseling, problem solving, oral communication, written communication, interpersonal relations, group presentations, adaptability, change management, systems thinking and team building.
Equivalent combination of education & experience (lesser) may be considered in lieu of requirements.
A skill assessment (e.g., typing, pharmacology, ward clerk) is required for this position. Candidates will be considered only upon passing the appropriate skill assessment.
COMMUNICATION AND MENTAL REQUIREMENTS
Talking to co-workers, customers, outside vendors, and on the telephone. Reading, writing, speaking, understanding English. Training/giving and receiving instructions. Mathematical ability, attention to
detail (e.g. organization, prioritization, proofing), concentration, and alertness.
PREFERRED QUALIFICATIONS
LICENSURE/CERTIFICATION
Valid Hawaii APRN Recognition with Prescriptive Authority (must meet education requirement(s) for Hawaii State APRN prescriptive authority).
Certified Diabetes Educator (CDE) by the National Certification Board for Diabetes Educators (NCBDE).
EXPERIENCE/OTHER
Three years direct patient care experience as an RN.
One year APRN experience in diabetes and related co-morbidities clinical services.
Clinical program development experience.
Demonstrated familiarity with statistical and analytical methods.
Demonstrated knowledge of PC and skill in the following PC applications: word processing, spreadsheet and database.
