Job Details:

  • Job Summary: The Case Manager-Outreach is primarily responsible for providing social services case management to a diverse population of sheltered and unsheltered homeless clients with multi-faceted and complex needs. Services are rendered primarily from the medical outreach van on the streets at night, as well as from the 5th Health Station at Christ House. This includes, but not limited to, forming therapeutic relationships with clients who are seriously and persistently mentally ill and/or actively using substances; assessing clients' needs; assisting clients in following through on service plans to meet identified needs, referral and linkage to community resources including health insurance and prescription assistance, advocacy, crisis intervention, and short term supportive counseling. Required Education/Required Training: High School Diploma or equivalent. Required Experience: Minimum of 5 years work related experience with a difficult population, preferred. Experience working with underserved or homeless populations preferred. * For correctional position-correctional experience- preferred but not required.
