We're looking for an RN with clinical, long-term care or home health and case management experience to join our new team. As the Community Based RN Case Manager, you would act as an advocate for our Medicaid members with the goal of keeping them happy and independent. Key responsibilities of the Community Based RN Case Manager include:
- Visiting members in their homes or other facilities
- Identifying early risk factors
- Conducting ongoing assessments
- Creating care plans
- Focusing on disease management and decreasing unnecessary hospitalizations
- Consulting with the primary care physician to deliver and coordinate necessary services
- Building relationships with members and their families

Qualifications Requirements include:
- A current, unrestricted RN license in Hawaii
- Clinical experience
- A background in long-term care or home health
- Familiarity with electronic documentation
- Basic computer proficiency and good typing skills
In addition, the following assets would be ideal:
- Case management experience strongly preferred
- Medicaid and/or Medicare experience
- Bilingual (Chinese, Filipino, Japanese, Korean, Samoan or Vietnamese) skills
- Pediatric experience desirable
