Position Summary:
Evercare Case Managers work in a variety of settings: in the community providing proactive care coordination to individuals and support to their families or in telephonic care management position. Many of the positions at Evercare offer the opportunity to create a flexible schedule so you can manage your caseload and your home-life schedule. It's the best of both worlds!
Primary Responsibilities:
- The Case Manager is responsible for clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating, providing health education, coaching and treatment decision support for members and their families.
- Drives quality, cost effective outcomes and minimizes fragmentation of health care delivery
- Works with consumers, caregivers, and family members to avoid admissions and re-admission to hospitals, emergency departments and other high cost services.
- Leads a collaborative process of assessment, planning, facilitation, and advocacy to meet member's health needs.
- Empowers the member to enhance their quality of life
- Motivates the member to positively change in behavior to promote optimal outcomes for all parties involved
- This is a telecommuter position after training, but must be located in Phoenix, AZ or Greensboro, NC.
Qualifications
- Current and unrestricted RN license from the state AZ or NC
- Undergraduate Nursing Degree
- 2-3 Case management experience
- Typing proficiency
- Strong computer skills in Word, Excel, Outlook & Internet
- Access to high speed internet for which the company will pay
