Job Description:
Performs evaluations, develops and implements effective patient treatment plans to restore, compensate or adapt for loss of patient function, by planning and administering medically prescribed therapy treatments in accordance with federal, state and professional standards governing the facility and their discipline.
ESSENTIAL JOB FUNCTIONS:
PERFORMANCE CULTURE:
§ Participates in activities related to identification of patients in need of therapy services.
Therapist Standards:
* Possesses broad geriatric clinical base allowing for identification of needs of patients. This includes patients with needs for functional maintenance programs and therapy courses that result in incremental changes, which assist in maintaining quality of life or maintenance of the highest practical level of function.
§ Conducts evaluations, assesses, develops, and implements treatment plans to meet goals and needs of patients.
Therapist Standards:
* Ensures evaluations and treatment plans are completed timely and in accordance with customer and regulatory standards.
* Includes patient/family in planning of treatment goals.
§ Effectively utilizes a transdisciplinary model.
Therapist Standards:
* Works with other disciplines to ensure patient outcome, continuity and carry over in therapeutic programming.
* Is knowledgeable of other discipline’s roles and refers as needed. Ensures duplication/inconsistency in therapeutic approach does not occur.
§ Develops an effective discharge plan in conjunction with the transdisciplinary team throughout LOS.
Therapist Standards:
* Is knowledgeable of discharge plan from time of admit through discharge and ensures patient and family are prepared for discharge as related to therapy issues.
* Completes home assessments when needed and uses assessment results in therapeutic programming. Notifies team of plans to discontinue treatment at least a week prior to actual discontinuation of services.
* Modifies therapy plan when discharge plans change.
§ Communicates treatment plans and progress to patient, family and caregivers.
Therapist Standards:
* Proactively includes all involved parties in the therapeutic process through phone contact, invitations to observe/participate in therapy, regular updates on progress/barriers, as appropriate.
§ Supervises assistants, aides, and students in accordance with state practice acts and local, state and federal regulations.
Therapist Standards:
* Possesses skills to supervise assistants, aides and students to most efficiently bring about optimal clinical outcomes for patients. Develops clinical skills in assistants/aides.
§ Ensures smooth and timely transition from skilled treatment to restorative/nursing programs, including documentation and teaching/training of caregivers.
Therapist Standards:
* Possesses skills to effectively teach and train all caregivers to assist patients in improvement and maintenance of function. This includes establishing functional maintenance programs that are understandable by nursing staff, and follow up to ensure continuity in programming post discharge from the therapy program.
§ Ensures patient rights and privacy are protected when delivering treatment and in discussions regarding patient at other times.
§ Maintains positive working relationship and demonstrates flexibility in problem solving with the team.
Therapist Standards:
* Promotes our core value of commitment to each other’s success. Respects, supports, encourages and inspires facility associates in all departments by being a strong team player and geriatric patient advocate.
* Promotes open lines of communications with other associates and customers. Seeks and gives candid feedback and provides opportunities for improvement through effective problem solving techniques. Avenues of effective communication might include:
Active participation in monthly all-associate meetings; teleconferences as requested by DM; timely and accurate documentation; informal, intrafacility communications, submission of articles to Beverly Cares on patient successes; staying informed about Beverly through reading the Inner Circle and attending facility inservices when appropriate.
* Supports district manager and facility in the orientation of new associates. Provides inservice education and mentoring. Through direct patient care activities, develops and shares best practices.
* Demonstrates role as a geriatric/LTC advocate by being involved in state and/or national professional organizations. This may be evidenced by one or more of the following: attendance at meetings; making presentations; participation on task forces or in special interest divisions; active in lobbying efforts; member of advisory or legislative council.
* Builds trust by carrying out assignments effectively and consistently.
* Actively utilizes clinical and operational resources at the district and corporate levels.
§ Uses clinical competency checklist to implement a plan for professional growth in conjunction with DM/RM. Shares knowledge with district and company to assist in developing or maintaining the clinical excellence of AEGIS Therapies.
Therapist Standards:
* Uses Clinical Competency Checklist to plan for professional growth, related to our core population. Works with DM to establish goals directed towards clinical excellence. Actively incorporates those goals into the performance plan and achieves goals as outlined.
* Attends continuing education courses in core clinical areas and demonstrates implementation of concepts learned as evidenced by treatment technique changes or changes in caseload.
* Gives input and involved in development and/or implementation of Clinical Best Practices.
§ Maintains a high level of customer service.
Therapist Standards:
* Identifies issues/concerns both in customer survey and in day to day interactions and works towards resolution in a timely manner. Resolutions are consistent with the policies of AEGIS Therapies and the customer. Informs DM of issues as they arise.
* Seeks opportunities to deliver exceptional customer service by continually seeking to understand the customers’ needs. Works to satisfy those needs. Recognizes that people have different backgrounds and experiences. Is open to new ideas and willing to modify own approach to best accommodate the customer. Communicates proactively with customer in regard to schedule, patient issues and therapy plans.
§ Proactively communicates with the District Manager regarding time away from work to ensure patient’s needs may be met in absences.
MARKET LEADER:
§ Communicates opportunities for new business development to DM/RM when known.
§ Willing to assist in startup/staffing of new business sites and products.
CREATE ECONOMIC ADVANTAGE:
§ Documents according to company policy, state and federal guidelines, and applicable accreditation agencies. Documentation is timely and supports the medical necessity of the skilled services rendered.
Therapist Standards:
* Develops skills to effectively advocate verbally and in writing for patients and profession from the very beginning of service delivery through the appeals process, if necessary. Documentation consistently supports medical necessity and skilled nature of the services.
*Participates in the in-house claims/medical record review. Resolves as part of a transdiciplinary team any barriers to maintaining coordinating documentation among disciplines.
§ Participates in appeals process as directed by DM/RM.
§ Completes all billing records, efficiency reports, payroll records or any other required reporting systems, timely and accurately, as per company standards.
§ Maintains safe and clean working environment by complying with facility and department procedures for infection control, OSHA regulations, and department maintenance/orderliness.
Therapist Standards:
* Maintains therapy area and supplies in a neat and orderly manner.
* Reviews infection control policies and OSHA Regulations regularly and consistently complies with them.
§ Secures equipment, adaptive devices and supplies in accordance with company policy and maintains adequate inventory. Operates equipment according to manufacturer’s instructions and reports malfunctions immediately to DM/RM and facility Executive Director.
Therapist Standards:
* Ensures adequate inventory available to meet patient needs. Informs DM of any issues that disrupt delivery of supplies/adaptive devices.
§ Assists DM/RM with non-patient specific time related activities as requested.
Therapist Standards:
*Is knowledgeable of the Scope of Service for a given facility and completes assignments in relationship to this.
* Accurately records optional services when delivered.
§ Is knowledgeable of and stays in compliance with all company policies, state practice acts, Federal guidelines (Medicare, Medicaid, VA) and rulings. This includes the items in the Quality Management Standards Handbook, Code of Conduct and incident reporting.
§ Will comply with AEGIS Therapies’ Code of Conduct, policies and procedures contained in company, Federal and State requirements and regulations.
MARKET MAXIMIZATION:
§ Establishes and fosters relationships with referral sources.
Therapist Standards:
* Maintains good physician/referral source relationships. Is an effective advocate for therapy services during physician interaction.
*Assists DM with marketing/sales and community relations activities as requested.
§ Manages the clinically appropriate therapy minutes per RUGS category for patients in conjunction with transdisciplinary team.
Therapist Standards:
* Works with Team Leader/MDS Coordinator in timely and accurate completion of the MDS items entering minutes in appropriate sections as required. Works as a member of the team to deliver outcomes within RUGS parameters. Develops understanding of the financial outcome of clinical decision making. Completes RST logs timely and accurately.
§ Utilizes cost-effective service delivery techniques when appropriate.
Therapist Standards:
*Develops skills to continually provide cost-effective service delivery without compromising quality of patient care. These include, but are not limited to, concurrent therapy, group therapy, rehab aide usage and efficient point of service documentation. Maintains efficiency at agreed upon level through use of these techniques and good time management.
Physical and Sensory Requirements (With or Without the Aid of Mechanical Devices):
Mobility, standing, pushing, pulling, reaching, bending, walking, heavy lifting, fine hand coordination, ability to hear, ability to read and write, ability to detect odors, and ability to remain calm under stress.
QUALIFICATIONS:
· Graduate from an accredited program for Speech-Language Pathology.
· Must possess and maintain a current license in the state of practice as required.
· Must possess problem-solving skills of the type and at a level necessary to accomplish the job.
· Must demonstrate good time management and organizational skills.
· Must be able to communicate in English, both verbally and in writing.
· Must be able to relate professionally, positively, and cooperatively with patients, patient’s family members and facility staff.
· Must be capable of maintaining regular attendance.
· Must meet all local health regulations, pass post-offer drug test, and post-offer employment physical exam if required.
· Must be capable of performing essential job functions of this job, with or without reasonable accommodations.
· Must have own transportation to travel between sites as needed.
· Must possess computer skills of the type and at a level necessary to accomplish specific job duties and assignments.
· Must meet Clinical Competency requirements upon hire and as needed. |
