Assist clients with functioning in the least restrictive environment. Responsible for assessing, linking, monitoring, and advocating for all assigned clients. Assures that all assigned clients received the most appropriate services available to meet their identified needs. Provide assessments, treatment planning, discharge planning, care decisions, and reassessments. Responsible for maintaining units of service (UOS) expectations. Completes all documentation and billing of all service provision.

REQUIREMENTS:

BS degree in social work, psychology, or Human services related field. At least one year in Mental Health or human service field related experience. Valid Florida Drivers License.
