Under the direction of the Nurse Supervisor, the Nurse Consultant will work as a practitioner and co-supervisor of social service staff highly trained as Assessment Workers involved in the assessment, Long Term Care level and locus of care determination and preliminary care plan development.
REQUIREMENTS: · Bachelor's degree in Nursing or Registered Nurse. · Must have a minimum of two years of nursing experience in a nursing home or community based setting or hospital discharge planning. · Experience working with social workers and serving the elderly or disabled populations is preferred. · Excellent interpersonal and communication skills. · Must be well organized, detail oriented and have the ability to function in a deadline driven environment. · High sense of professional ethics. · Valid driver’s license with a good driving record. · Pennsylvania RN License.
DUTIES AND FUNCTIONS: · Review and approve assessments, level of care determinations and consumer care plans completed by Assessment Workers; make recommendations concerning their appropriateness in addressing an individual consumer’s long term care needs. · In conjunction with an Assessment Supervisor, supervise Assessment Workers and a Clerk typist through team meetings, case reviews, joint supervision, field observation and day to day contact. · Work with Assessment Supervisor in resolving any issues and in completing performance appraisals for staff. · Initiate case conferences with Assessment Workers, Supervisors, Program Medical Consultants and other disciplines regarding level of care decisions and care plans. · Provide direct health assessments when appropriate. · Prepare written testimony and participate in appeal hearings. · Perform standardized record keeping, prepare monthly Referral Reports and other written reports as assigned. · Serve as resource person for health coverage and benefits and make recommendations regarding potential third party coverage for services. · Advocate on behalf of our consumers at risk of institutionalization. · Evaluate the availability and appropriateness of community based long term care programs or facilities to best meet applicant's needs and makes the most appropriate, least restrictive, focus of care decision with consideration of applicant's preferences. · Investigate and develop health resources and disseminate relevant information to staff. Work to integrate health and social services on a system as well as a consumer specific level. · Assist in the development of orientation and training of staff. Provide and/or coordinate in-service training and plan or participate in other training as directed by Nurse Supervisor or Director. · Participate in ongoing problem solving, program evaluation and staff development and make recommendations to the Nurse Supervisor and Director to resolve issues or problems. · Provide back up to and work with departmental Nurse Consultants and Supervisors to assure the department’s goals and objectives are met. · Assist in the interviewing and hiring of staff. · Participate in monthly Quality Assurance reviews of assessment case records and assist in preparation for audit and monitoring reviews. · Perform other duties as assigned.
