Responsible for the implementation of individualized Day Rehabilitation and other Program Services as described in the Individual Service Plan and Individualized Client Plan with the purpose of developing each individuals opportunities for self-determination, independent activity, increased productivity and for greater community inclusion. May function as a Team Leader for each consumer assigned. Participates in all aspects of direct care for consumers assigned.

Responsible for the direction of Skills Specialists and all temp. or substitute staff assigned to their program area.

High School diploma or equivalent required.

At least 1 year of related experience, specifically working with Developmentally Disabled and/or medically frail individuals.

Must be at least 18 years old and be able to lift a minimum of 50 pounds
