Current Calif RN Lic. required. Masters prepared as a CNS with minimum five years geriatric experience. BLS cert. required prior to clinical orientation. Demonstrates knowledge of advanced nursing theory and clinical proficiency specific to geriatrics/dementia care.
Clinical specialty in geriatrics preferred. Knowledge of community resources for seniors preferred. The Dementia Care Clinical Nurse Specialist functions as an expert clinician to address the needs of patients with Alzheimer's disease (AD) or a related dementia and their families house-wide. Collaborates with all members of the interdisciplinary team to assess and respond to cognitive, psychiatric/behavioral, and functional symptoms indicative of dementia, delirium, or a related condition. Functions as a clinical expert, consultant, educator, and leader to plan patient care and ensure optimal patient care outcomes. Provides after-hours consultation in the management of patients with AD/dementia. Participates in discharge planning to specifically adress the dementia-related needs of these patients and their families, and facilitates access to community-based resources. Establishes, captures, and analyzes outcomes to evaluate effectiveness of and improve patient care. Assists with inter-departmental preparation for JACHO (i.e., clinical competencies and policy and procedure standardization). Participates in the continuous quality improvement process.
