Position Summary

In this position, the Weight Management Educator is responsible for education and advisory functions to patients within the Weight Management Clinic as well as being directly involved with patients in education programs related to weight management, individual education sessions related to weight management, cooking classes, and other activities that assist patients in weight loss, maintaining weight loss and changing lifestyle behaviors.

Essential Duties and Responsibilities

Weight Loss Education

  • Develops individualized needs assessments for patients participating in the weight loss program.
  • Works collaboratively with patient and Medical Director to develop a plan for weight loss. Implement, evaluate and revise the food exercise and social parameters of the weight loss experience for individual patients.
  • Plans, coordinates and teaches individualized and group education based on program protocols and the results of the patient needs assessments.
  • Documents all aspects of the education services received by the patients or other relevant aspects of the patients’ education experience on medical record.
  • Provides ongoing program education and follow-up services to patients who have enrolled in the weight management program.
  • Supplies appropriate consumer health/patient education materials to patients and their families.
  • Plans, coordinates, facilitates and participates in community outreach activities and events such as health fairs and the SVH Done in a day programs

Quality Improvement

  • Identifies risks, promptly investigates problems and complaints, and takes action as appropriate to improve service.
  • Participates in meeting Joint Commission standards and other accrediting organization requirements.
  • Supports and promotes interdepartmental collaboration and recommends innovative programs and structures that will improve quality of patient care and educational programs.

Education, Experience and Licensure/Certification Requirements Requirements are

representative of the minimum level of knowledge, skills and/or abilities necessary to perform the essential functions of the position

  1. Bachelors degree in education, science or nutrition
  2. Minimum of one year experience in working with patients and their applicable weight loss programs
  3. Current BLS certification or ability to obtain within three months of hire
  4. Demonstrated teaching experience in a patient education program.
  5. Ability to communicate effectively and diplomatically within a multi-functional team
  6. Strong organizational skills and attention to detail
  7. Ability to successfully function in a fast paced, service oriented environment

Experience in understanding and usage of computers, including the Microsoft Office Suite, as well as the ability to learn applications relevant to the postion
