Responsibilities:
  • Oversee gathering of information for assessments and care plans; schedule and facilitate care plan conferences
  • Monitor and guide the completion of PPS, OBRA assessments and RAPS
  • Assess prospective residents in conjunction with other team members
  • Perform the functions of a nurse when required
  • Handle job responsibilities in accordance with the Company's Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standards and applicable state/federal laws.

  • A candidate for this position should be accurate, concise, detail-oriented and able to work cooperatively as a member of a team. Also needed is the ability to organize, prioritize and communicate effectively with residents, their families and all levels of the organization. Should react decisively and quickly in emergency situations and maintain confidentiality. Knowledge of RAI requirements is required. We are looking for a graduate of an accredited School of Nursing (RN preferred) with a valid state nursing license and CPR certification. One year of nursing experience is preferred.
