Position Summary:
The Care Coordinator will manage the care of members in their homes or other community settings telephonically, focusing on disease management and keeping members healthy and independent.
Primary responsibilities:
  • Engaging members telephonically to coordinate services, community resources, and treatment needs.
  • Delivering a holistic approach to coordinated care based on the enrollee needs
  • Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file.
  • Creating, reviewing and revising Plan of Care plans and focusing on disease management.
  • Functioning as an advocate for members and decreasing unnecessary hospitalizations.
  • Collaborating with the member's PCP to deliver and coordinate necessary services.
  • Building relationships with members and their families; assisting them with proper health choices
  • Ensuring cases are documented in a timely manner.

Qualifications

Qualifications:
  • Bachelor's level degree in Social Work or other related field
  • Minimum of 1-3 years clinical or Case Management experience
  • A background in long-term care (home health, hospice, public health, assisted living)
Additional Assets Preferred:
  • Medicare/Medicaid experience
  • Strong understanding of Community Based Resources and National Programs
  • A background working in geriatric special needs
  • A background in behavioral health
  • Experience with electronic charting
  • Ability to multitask
  • Computer Proficiency
