OHS is seeking an RN Case Manager, full time, for our Pediatrics Division. Responsibilities will include: overseeing and coordinating patient’s plan of care with the Multi-disciplinary team. Performing comprehensive OASIS assessments and providing skilled nursing services in accordance with Physician’s order. Providing summary reports to the patient’s physician if the patient’s condition changes. Clinical record reviews, focusing on regulatory compliance and case management. Must have 1 yr Case Management experience. Pediatric experience a plus.
