Job Description: Clinical Pharmacist Provides highly specialized and complex pharmaceutical services to improve drug usage and therapeutic outcomes for persons with developmental disabilities residing at a State Supported Living Center (SSLC). Conducts Drug Utilization Evaluations (DUE)/Drug Utilization Reviews (DUR) on commonly prescribed medications at the SSLC. Analyzes and documents assessments of residents' pharmaceutical regimens at regular intervals, including issues related to use of antibiotics, anti-convulsants, psychoactive medications, and polypharmacy. Participates in Interdisciplinary Team focused on the clinical care of residents at SSLC including Health Status Team meetings and ad hoc Interdisciplinary team meetings. Provides continuing education to pharmacists, physicians, and nurses on issues concerning drug therapy, the inherent toxicity of drugs and side effects, as well as assisting in the prescription of appropriate doses. Serves on facility committees and workgroups which have a pharmacy related component. Functions independently with minimum supervision and extensive latitude for use of initiative and independent judgment within the scope of practice outlined by the Texas State Board of Pharmacy. |

Essential Job Functions: Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned. 1. Documents Quarterly Drug Reviews on all clients at SSLC to include, but not limited to, the following areas: A) Appropriate use of PRN (as needed) medications and discontinuation of medications due to non-use; B) Assure laboratory values and drug levels are drawn as indicated and receive appropriate follow-up; C) Assure that appropriate vitamin and folic acid supplements are prescribed for persons on chronic anticonvulsant therapy; D) Assure residents are on appropriate Osteoporosis treatment or prophylaxis (including age recommended daily allowances of Calcium supplements); E) Identify medications that may pose an absolute or relative contraindication to existing medical conditions conferring with primary physician when appropriate; F) Monitor for pertinent drug allergies; G) Monitor for potential drug-drug, drug-food, and drug-disease interactions; and H) Identify and address Polypharmacy, (25%) 2. Performs focused drug reviews on persons receiving psychoactive medications, including, but not limited to A) Monitor for potential drug-drug, drug-food, and drug-disease interactions; B) Identify and address Polypharmacy; C) Collaborate with Staff Psychiatrist to ensure appropriate parameters (laboratory tests, weight, EKG/diagnostic tests) are being monitored as recommended by evidence based standards. At a minimum, attends Behavior Support Plan team meetings, Psychoactive Medication Management meetings, and Neurology Clinic as needed. (25%) 3. Attends and participates in Health Status Committee meetings for SSLC residents. (25%) 4. Conducts Drug Utilization Evaluation/Drug Utilization Reviews (DUE/DUR) at least quarterly on commonly prescribed drugs and prepares a verbal and written report for the facility’s Pharmacy and Therapeutics Committee. Utilizes the DADS/DSHS Executive Formulary Committee’s Medication Audit Criteria and Guidelines or comparable evidenced based audit criteria to conduct DUE/DUR audits on client records. Remains current on evidenced based pharmacy practice and incorporates appropriate current literature into the DUE/DUR process. Uses existing databases (WORx and CWS) reports as basis for DUE audits. Serves as Liaison between DADS/DSHS Executive Formulary Committee and the facility at a minimum monitoring the Minutes of DADS/DSHS Executive Formulary Committee and assuring that all required facility pharmacy reports are submitted to the DADS/DSHS Executive Formulary Committee in a timely manner. May represent facility on DADS/DSHS Executive Formulary Committee and/or other state workgroups related to clinical pharmacy practice upon request. (5%) 5. Tracks and trends all adverse drug reactions (ADR) at facility assuring that they are reported to the DADS /DSHS Executive Formulary Committee and FDA as appropriate. Reports summary report of Adverse Drug Reactions to facility’s Pharmacy and Therapeutics Committee. Serves on the campus Infection Control Committee. Serves as consultant to physicians on specific infectious disease clients. Assesses and trends antibiotic resistance at facility at least annually. (5%) 6. Provides inservice training to nurses, physicians, and staff pharmacists and assists in the preparation of inservice programs for other facility staff. May serve as preceptor for Pharmacy interns. Participates in continuing education activities as necessary to remain current on new pharmaceutical products, new evidenced based guidelines for pharmacy practice, and new developments in the treatment of conditions and diseases commonly found in the facility’s population. (5%) 7. May provide emergency backup to staff pharmacists, including the supervision of Pharmacy Technicians, to assure the lawful dispensing of medications at SSLC. (5%) 8. Knowledge Skills Abilities: 1. Extensive knowledge of drug products, medical therapeutics, pharmacokinetics, and disease states. At least 2 years experience in a clinical setting is desirable, but not required. 2. Ability to communicate clearly in oral and written form. 3. Ability to work with information systems, automated, and technical equipment. 4. Ability to utilize Microsoft Office Suite (Word, Excel, Access, and Outlook) to prepare all written communications. 5. Knowledge of on-line/internet based reference materials and skills for conducting focused Literature Reviews related to the practice of clinical pharmacy. 6. Ability to develop and maintain liaison relationships with medical and nursing staff providing timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, etc. 7. Skills in conducting and participating in Drug Utilization Evaluation/Drug Utilization Reviews (DUE/DUR) or comparable medical research or Quality Assurance reviews including basic knowledge of statistics and demonstrated ability to collate and analyze data. 8. Knowledge of Texas State Board of Pharmacy rules, Federal Drug Administration regulations, and other comparable rules/regulations related to the practice of pharmacy within the State of Texas and CMS/DADS certified ICF-MR facilities. |

Registration or Licensure Requirements: Licensed Pharmacist by Texas State Board of Pharmacy. Doctorate in Pharmacy generally preferred, however any of the following are acceptable: B.S., M.S., PharmD, and PhD. |
