Clinical Liaison
Overview:
To engage in assessing and evaluating patient’s in acute care settings that meet the criteria for admission to an ArchCare facility. Evaluates the medical and clinical healthcare needs and assists with the coordination and planning of all possible referrals made to all ArchCare facilities. Monitors and facilitates communications regarding medical conditions of all ArchCare patients in the acute care hospital that are on bed hold or have been admitted without bed hold.

1. Assess the physical, functional, social, psychological, environmental, and financial needs of clients/patients at the acute care hospital via the Patient Assessment Form.
2. Identify a cost-effective plan to meet facilities’ needs.
- Facilitates communication with acute care hospital staff to ensure that services needed can be delivered and met at the facility.
4. Assess the client’s formal and informal support systems.
5. Maintains close communication with admissions department at the Archcare facility in regards to bed availability.
6. Act as facility advocate; identify and assist with placement of possible admissions.
7. Must have knowledge of Patient Review Instrument; certified PRI assessor preferred.
8. Assist hospitals in expediting the placement for potential admissions to an ArchCare facility.
9. Maintains patient confidentiality.
10. Meets with patients/families and educates them regarding the unique services offered at the ArchCare facilities.
11. Perform related duties and responsibilities as required.
12. Collaborates with patient or representative and interdisciplinary team to ensure seamless transfer to the facility, including the discharge plan.
13. Acts as the single point of decisions making for admissions with collaboration as needed.
Qualifications:
- ArchCare Mission, Vision, and Values
- Principles, procedures, methods and techniques of nursing, rehabilitation, and health care preferred.
- Modern practices and principles of medical disease counseling.
- Basic management practices and principles.
- Admissions policy and procedures.
- ArchCare facility service offerings
- Payor sources.
- Excellent computer skills preferred knowledge of Microsoft excel/word/outlook.
- Knowledge of HMO contracts and levels based on medical needs.
- Universal precautions and infection control practices.
- Pertinent Federal, State, and local laws, codes, and regulations regarding health care.
Requires RN, BSN.
