Diabetes Education Specialist
The University of Minnesota Medical Center, Fairview has a full time opening for a Diabetes Education Specialist at our Riverside Campus in Minneapolis , MN . This individual will provide education to patients, their families, and appropriate systems about diabetes self-management and related issues, enabling them to manage a chronic disease. Education includes assessment, goal setting, intervention, and evaluation follow-up and support. The qualified candidate will have a strong knowledge base of diabetes, pathophysiology of disease, strong assessment skills and the ability to coach patients and encourage their progress. Must also have a strong knowledge base of adult learning principles and the ability to adapt learning styles to meet patient needs as well as up-to-date knowledge of best practice in the treatment and management of diabetes.
