Assistant Case Manager: Shawnee Health Service

POSITION: Social Service I- Assistant Case Manager

RESPONSIBILITIES: The ACM at AHC/SHC-C will work primarily with the Primary Care and Mental Health Integration Project through the length of the project. The ACM will work closely with the Clinical Care Manager and Case Managers as the Integration project continues. The ACM will be responsible for the data entry of information on referrals from primary care for patients that need mental health services into a patient registry. This Registry will track patients throughout all SHS clinics. The ACM will also assist patients with applying for SHS’s SFAP and Real Benefits screenings, PCAP etc. and will have cross training on other PSR related duties. This position may move to the SHC-C site when it opens or may split time between both clinics depending on clinic needs.

QUALIFICATIONS: An Associate degree in a human services field is preferred. Knowledge of SHS center programs and other community services is required. Experience with Mental Health services would be beneficial.
