The Care Coordinator Social Worker is part of the care management team and will provide telephonic care management to individuals who live in their own homes or community settings. The purpose of this role is to help keep peoples healthy and independent.
Primary responsibilities:
  • Engaging members telephonically to coordinate services, community resources, and treatment needs.
  • Delivering a holistic approach to coordinated care based on the enrollee needs
  • Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file.
  • Creating, reviewing and revising Plan of Care plans and focusing on disease management.
  • Functioning as an advocate for members and decreasing unnecessary hospitalizations.
  • Collaborating with the member's PCP to deliver and coordinate necessary services.
  • Building relationships with members and their families; assisting them with proper health choices
  • Ensuring cases are documented in a timely manner.

Qualifications

  • Requires a current, unrestricted license as an RN, LGSW or LICSW
  • Minimum of 1-3 years clinical or Case Management experience
Additional Assets Preferred:
  • A background in long-term care (home health, hospice, public health, assisted living)
  • Medicare/Medicaid experience
  • A background working in geriatric special needs
  • Experience with electronic charting
  • Previous Medical Case Management and/or discharge planning experience
  • Computer Proficiency
