Summary: Responsible for directing the entire unit in a long term facility

Qualifications:

Education:

§ Graduate of accredited school of nursing, BS degree preferred.

Licenses/Certification:

· Licensed as a LPN/RN in state employed.

· Valid CPR teaching certificate, if applicable.

Experience:

§ Three years of nursing experience, of which one year was in a long term care environment.

Essential Functions:

§ Conducts Core Program/Assessments in accordance with Standard of Care:

§ Falls, Bowel and Bladder, Psychotropic Medications, Restorative, Pain Management

§ Skin, Behavioral, Nutrition and Hydration.

§ Lead Accident and Incident trending

§ Manage Infection Control

§ Manage Restorative Nursing

§ Ensure follow-up and recommendation with: Dietary, Pharmacy, Lab, Rehab and Psychological

§ Complete staff management of the unit to include, interviewing, hiring, and discipline. Scheduled time off and evaluations

§ Ensure effective overall staff utilization that includes responsibility for all shift staffing.

§ Participate in Quality Assurance Program

§ Management of Medication program for the unit

§ Identify training needed for unit staff

§ Participate in new staff orientation

§ Conduct Focus Rounds

§ Be the primary contact for all unit patients and families and ensure effective and professional communications

And prompt problem solving

§ Ensure Units are prepared for new patient move-ins

§ Participate in facility specific on-call rotation

§ Participate in all required meetings as Unit Director of Human Resources

§ Performs other tasks as assigned.

Knowledge/Skills/Abilities:

§ Skilled in motivation of staff and leadership.

§ Skilled in work assessment.

§ Ability to communicate effectively with residents and their family members, and at all levels of the organization.

§ Knowledge of state and federal requirements for accreditation and certification.

§ Possesses basic computer skills

§ Ability to work with a culturally diverse resident and employee population.
