Geriatric Care Manager
Caritas Home Care / Norwood Office

Responsibilities:

  • Provides superior customer service to internal and external clients, customers and patients as referenced in the Service Excellence Standards for AEC.
  • Incorporates administrative and clinical performance improvement into daily program operations.
  • Ensures regulatory compliance to all Federal, State and Joint Commission rules and regulations.
  • Identifies and resolves issues impacting program services.
  • Assesses staffing for program services at all branches.
  • Acts as a resource to clinicians and home health aides.
  • Adheres to fiscal responsibility for the program.
  • Allocates resources appropriately.
  • Actively participates within the clinical leadership team

Requirements:

  • Must possess a current Mass. nursing license
  • Must possess current, valid driver's license
  • Graduate of an accredited school of nursing
  • Minimum 1-2 years experience in certified home health operations
  • Must have demonstrated competence and satisfactory performance with direct patient care in a community health or medical/surgical setting for a minimum of three years.
