Monday through Friday - 8 am to 5 pm
(occasional weekend work)
What a great opportunity to get OFF THE FLOOR and work in a nice office environment as you will be trained on telephonic disease management practices. This superb job has an opportunity to WORK FROM HOME after training and several months of proven success.
Responsibilities include assessment and evaluation of patient and the data provided for each patient. Complete interventions as necessary and sending reports to the physician. Identify patient learning needs and abilities and document all communications regarding patient in the their record.
