Job Title: Family Outreach Worker

Description:

  • MAJOR FUNCTIONS:

    The Family Outreach Worker conducts outreach and chemical dependency screenings in community based organizations and refers pregnant and postpartum women into the HMHF program for assessment and treatment.

    SPECIFIC DUTIES & RESPONSIBILITIES:

    s Networks with social service program administrators for effective coordination of outreach services and establishes linkage agreements with the programs within Northern Manhattan and the Bronx who serve pregnant and post-partum women;

    s Delivers educational presentations on chemical dependency, mental health problems and problems and available services to pregnant and post partum women to program staff from identified social service programs within Northern Manhattan and the Bronx ;

    s Develops monthly calendar of outreach activities;

    s Coordinates outreach and screening activities for chemical dependency and mental health problems, and refers clients who screen positive for appropriate services;

    s Follows up on outcome of service referral;

    s Prepares statistical reports to monitor effectiveness of outreach efforts; and

    s Participates in in-service training as directed by supervisor.

Requirements:

  • REQUIREMENTS:

    s Two years of direct service experience in treatment of chemical dependency and mental health disorders including one year experience working with pregnant and post partum women;

    s Must hold a New York State CASAC-T certificate;

    s Well organized, communicates well, and able to multi-task;

    s Frequent local and some out of state travel required, valid NYS drivers license preferred;

    s Knowledge of local social services and substance abuse treatment programs which serve pregnant and post-partum women;

    s Fully adept at culturally and ethnically appropriate service delivery for a diverse target population;

    s Competent in use of identified screening instruments and protocols; and

    s Effective interpersonal skills, good platform skills, and fluent in Spanish
