Be the first point of contact for all new admissions. Screen all referrals for appropriate placement. Establish and maintain strong linkages with medical providers, hospitals, nursing homes and other potential referral sources to build awareness of St. Luke s Home & Rehab Center. RN, LPN or SW required.
