General Summary: Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician's plan of treatment, taking into account individual patients' special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential.
Required: Must have in-depth and up-to-date knowledge of physical therapy, theory and practice and the legal and ethical issues associated with them. Minimum of a Certificate, Bachelors or Masters degree in Physical Therapy. Physical Therapy licensure through the State of Oregon. Valid driver's license, automobile liability insurance, and reliable transportation. Minimum 1 year clinical experience per the State of Oregon Home Health Regulations.
