Physical Therapy Assistant/Home Health/South Service Territory Qualifications: High school graduate and graduation from approved program with an A.S. degree in applied science as a P.T.A. Illinois State License. If incumbent is not certified, he/she will be required to become certified within 6 months of employment. Job Description: Administers Physical Therapy treatment (ie. Modalities, massage, exercise) per M.D. order under the supervision of the physical therapist for the purpose of direct patient care. The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Unit team and as such meeting individual performance expectations and contribution to team performance goals is required. Responsible for administering and instructing the patient/caretaker on the therapeutic treatment program developed by the physical therapist (i.e. gait training, balancing, and coordination activities, bed mobility, w/c mobility, stair climbing training), in accordance with the plan of treatment. 5. Responsible for documenting patient treatment administered and patients response to treatment on a regular basis per policy and procedures of the agency. 6. Observation of the patients tolerance to treatment and therapy progress, discussion of patients condition, progress and treatment with supervising physical therapist. 7. Responsible for maintenance and development of skills as well as acquisition of agency information. 8. Responsible for generation of patient care revenues directly related to productivity, determination of appropriate frequencies, with adherence to frequencies. 9. Responsible for maintenance of agency licensure directly related to incumbents adherence to multiple regulatory policies including Illinois Department of Public Health Rules and Regulations, Medicare Conditions of Participation as well as JCAHO Home Care Standards.
