-100% position at San Antonio Health Center in Oakland with full fringe benefits,
-50% position at Vallejo Medical with partial benefits,
-50% position at Pittsburg Medical with partial benefits,
- OR a combination of both 50% Pittsburg and 50% Vallejo which would equal 100% and eligibility for full fringe benefits.


THE POSITION: The Behavioral Medicine Specialist (BMS) is a key member of the Primary Care team. The BMS provides behavioral health care consultation to the medical providers and their patients and is responsible for working in conjunction with the primary care provider to meet the medical and behavioral health care needs of the patient. The BMS performs mental health screening, assessments, diagnoses, brief interventions, and co-manages the behavioral health needs of the patient with the provider during the intervention period. S/he facilitates patient skill training through psychoeducation and behavior change strategies and develops specific behavior change plans with patients. S/he participates in interdisciplinary teams, program development and program evaluation to effectively promote access of mental health services in the medical setting.

MINIMUM QUALIFICATIONS:
1. Requires a Masters degree in Social Work form an accredited college or university and current license as an LCSW in the State of California, or PhD in Psychology and license as a Psychologist in the State of California.
2. Specialized training and experience in the principals of integrating behavioral health assessment and treatment in the primary care setting.
3. Experience in consultation/collaboration with medical providers.
4. Experience in Behavioral Health Crisis Intervention and Behavioral Health Triage.
5. Expertise in Mental Health and Substance Abuse screening, assessment and diagnosis.
6. Knowledge of Behavioral Medicine and the impact of physical health issues on behavioral health conditions.
7. Knowledge of evidence based treatment for medical and behavioral health conditions.
8. Knowledge of psychopharmacology.
9. Knowledge of chronic disease and the use of behavioral health interventions with chronic disease.
10. Expertise in brief cognitive and behavioral interventions.
11. Knowledge of community resources and patient eligibility criteria.
12. Ability to work efficiently and as part of team in the primary care setting.
13. Ability to establish and maintain effective working relationships with others.
14. Ability to accurately document clinical assessment, intervention, and plan in the medical record.
15. Computer literacy to retrieve patient information, input information, and search to web.
16. Bilingual in English/Spanish required.

DUTIES AND RESPONSIBILITIES:
1. Consult and collaborate with primary care clinicians regarding the treatment of behavioral health disorders, psychological and psychosocial problems.
2. Perform behavioral health screening, triage, evaluation, and assessment to determined appropriate level of behavior health care.
3. Assist in the detection of at risk patients to assess and provide care to prevent further psychological or physical deterioration.
4. Work with the primary care team to effectively and efficiently treat, manage, and counsel patients with chronic emotional and/or health problems.
5. Assist the primary care clinicians to develop and treat chronic pain and chronic disease and the related psychological and psycho-
social issues.
6. Provide crisis intervention and brief behavioral health intervention
7. Provide group interventions (treatment groups, psycho-education groups, self-management groups).
8. Provide education and support to patients and their families to encourage compliance with medical and behavioral health issues.
9. Plan, coordinate and facilitate referrals to existing physician time and develop appropriate patterns of utilization.
11. Provide consultation and training to other clinic staff in the area of behavioral health.
12. Develop and maintain procedures, protocols, and systems on an on-going basis to ensure the programs relevance , usability and sustainability for patients and clinic staff and funders
13. Participate in program evaluation, collecting data, providing input and information as community resources when indicated by assessed need.
10. Work with providers to aid in the management of medical conditions through the use of non-pharmacological and behavioral interventions in order to leverage needed.
14. Other duties as assigned.
