Nurse Case Manager

Responsibilities

Use the nursing process to provide case management services to an assigned caseload. Complete initial client/family assessments and develops problem list. Accurately identify and prioritize at risk patient populations. Perform home visits and phone follow up as indicated. Collaborate with client, physicians and other providers to develop a comprehensive plan of care. Effectively communicate care plans and other client-related activities both verbally and in writing to appropriate personnel and agencies. Utilize clinical skills and assessments to work autonomously in the outpatient community settings; communicate with physicians and other providers concerning alterations in the plan of care. Demonstrate the ability to use teaching, learning and counseling skills. Act as a client advocate for the development of community resources. Consult others to identify potential community resources for resolving client health, psychosocial, or financial problems. Function as a liaison to external agencies. Collaborate effectively with departmental/program staff to achieve goals and objectives. Maintain productivity and optimum client caseload as negotiated with department director. Ensure collection of data for outcomes and other studies as requested. Provide information for maintenance of case management client database. Support efforts to implement disease management model. Provide leadership in areas of clinical expertise. Demonstrate effective interpersonal communication skills. Develop and implement programs/projects which facilitate the attainment of corporate goals. Facilitate productive meetings with interdisciplinary team members from across the system. Develop strategic long range plans in area of specialization. Participate in financial planning for specialty care. Maintain and advance knowledge and skills in area of specialization. Participate in professional organizations. Participate in quality improvement (QI) activities. Change practice behavior as a result of data. Encourage clinical problem identification and resolution activities. Identify, address, and report quality issues identified through data sources.

BS in Nursing or related field required; Master's degree preferred. Must have current NC RN License. For School Health Services, certification from National Assoc of School Nurses (NASN) required within three years from date of employment. 3-4 years related experience required. At least 2 years experience in direct client care to acute and chronically ill clients in outpatient and inpatient settings. Evidence of progressive leadership and advanced teaching & communication skills essential. A Master's degree in Nursing, Education or Business may be substituted for one year of required experience; an additional year of related experience may be substituted for a year of the required education with a maximum substitution of two years.
