Seeking Social Worker that has worked in an non profit enviroment and worked with elderly.

LaJoy Group is Seeking Social Workers with the following experience:

Must have a Bachelors in Social Work and limited or full state licensure.

Candidate must have 2 years experience in a non-profit environment as well as 2 years experience with Senior Citizens.

Some experience with data entry and data management required.

Requirements:

Has excellent interpersonal and group skills.

Is accurate, conscientious and flexible with attention to detail.

Is familiar with computers.

Accepts direction, is well organized, and has the ability to meet deadlines.

Has good handwriting.

Willingness to work in a team environment.

Must have a valid driver's license and reliable transportation.

Duties:

Conducts comprehensive client assessment to determine the social, emotional and limitations of community care department (CCD) clients.

  • Work with the team nurse to develop the client care plans, including determination of frequency and duration of social services required under the care plan.
  • Facilitates delivery of social services determined necessary under the care plan with appropriate social service agencies.
  • Monitors client care plans to ascertain delivery of needed services, and performs client reassessments functions to determine needed alterations in the client care plans.
  • Works with family members and other volunteer caregivers to maximize available informal support systems.
  • And handles other duties assigned by the Community Care Department Manager, Director of Operations, and/or the Executive Director.
