What We Do:

HeartShare Human Services of New York was founded in 1914 as an after-care agency for young adults aging out of orphanages. HeartShare helped them face the harsh realities of life-on their own-from finding a home to getting a job. As society’s needs have changed, so has HeartShare.

Today, HeartShare serves 17,000 children, adults, and families in more than 60 programs. Its services for individuals with developmental disabilities include: early childhood services for children ages 3-5, Universal Pre-Kindergarten for children age 4, a school-age program for children ages 5-14 with autism spectrum disorders and mental retardation, recreational opportunities for teens and adults, adult day services, residential services for adults, family support services, case management, and health and mental health services.

Additionally, HeartShare offers foster care and adoption services, after-school youth programs, family counseling, services for those affected by HIV/AIDS, and energy assistance programs.

Its mission is to nurture and support, with dignity and respect, children, adults and families in order to expand opportunities and enhance lives. Approximately 90% of all revenue goes directly to program services. FOSTER CARE CASEWORKER

Job Description:

The major responsibility of the Caseworker is to assess and evaluate potential foster parents. This includes interviews with applicant(s), applicant’s family members, references, and emergency child care providers. In addition to interviews, home visits to the applicant’s residence must be made, to ensure that the home is in compliance with N.Y.S.D.S.S. and PAS regulations.

Responsibilities:

  • Assesses applicants interested in providing foster care system, foster parent role and responsibility as it pertains to HeartShare and to the children placed in their home.
  • Interviews child care alternates (babysitter(s) – emergency people), significant others and references
  • Provides foster parent applicant with information about foster care system, foster parent role and responsibility as it pertains to HeartShare and to the children placed in their home
  • Maintains case records, written reports and forms that are required by HeartShare, CWA and N.Y.S.D.S.S. Written reports include chronological entries of progress notes, correspondence to foster parent applicants, home study reports, closing summaries. All reports must be accurate, complete and submitted in a timely fashion
  • Attends and actively participates in supervisory and team meetings, family conferences, inter-disciplinary case conferences, and all meetings that are case related
  • Attends mandated training as well as other learning opportunities

Education:

  • Bachelor's Degree in related field required

Experience:

  • Child welfare experience preferred
  • Must be able to work with children and adults as well as conduct home visits and court appearances
  • Driver's license required
