We are searching for a nationally certified Nurse Practitioner to join our team. In this field-based NP role, you would provide a holistic approach to caring for our aging residents in long-term care facilities. Key responsibilities of the Nurse Practitioner include:
- Providing direct care to aging residents in long-term care facilities
- Collaborating with primary care physicians, facility staff and patients' families
- Participating in care conferences with other providers and patient family members
- Working with the facilities utilization review process
- Identifying patients' behavioral health issues
- Educating nursing home staff, physicians and administration
- Coordination of care for patients
Qualifications
Requirements:
- Nationally certified Nurse Practitioner status
- Certification in Adult, Family or Gerontology
- Current unrestricted license from IN
- Proficiency in Computer Skills
- Minimum of 2 years experience in long-term care
In addition, assets that would be beneficial include:
- Clinical experience
- Knowledge of mental health issues
- A background in managed care
- Familiarity with Medicare and ICD-9 coding
