Essential Duties:
May make initial evaluation visits and admit qualifying patients to service. Develops and implements the patient’s plan of care according to the physician’s orders. Coordinates services with the community agencies to provide proper patient care. Is responsible for recertification in a timely manner and documents all changes/communications in the plan of care and sends it to the physician for his/her signature. Arranges and coordinates all ancillary services and assures that Case Conferences are performed on a timely basis with all services participating. Provides competent nursing services in the patient’s home and observes signs, symptoms, and reactions to treatment and changes in patient’s physical/emotional condition and reports to the physician as necessary.
Teaches, supervises and counsels the patient/family regarding the nursing care needs and other related problems of caring for the patient in the home Plans and coordinates weekly schedules or makes visits as assigned. Prepares and reviews Home Health Aide care plans with the proper updates as necessary and makes supervisory visits as required by agency policy, documenting same. Properly completes documentation of the Skilled Nursing Visit Note and meets all the paper flow guidelines. Prepares discharge summaries for closing of patient files. Maintains knowledge of all applicable Federal, State, and Local laws and complaint procedure requirements. Observes all scheduled time frames for submitting accurate required paperwork in order to facilitate operations.
Complies with OHS policies and procedures. Participates in staff meetings and In-service Education programs as required. Seeks opportunities to enhance professional growth.
Skill Required:
Must be a registered nurse, currently licensed in the state of Texas. Shall have at least one-year experience. Demonstrates ability to provide nursing services in the home.
Minimum Qualifications:
Licenses/Certifications/Registrations: Registered Nurse currently licensed in the State of Texas. Maintains current Texas driver’s license.
