Clinician (#003FT) ID Case Management - Intellectual disability case management offers fulfilling employment providing services to adults diagnosed with intellectual disabilities in an outpatient setting. Primary responsibility is to provide direct service contact to link individuals to needed resources. Additional responsibilities include facilitating person-centered planning meetings as well as completing required documentation within specified time frames. Position requires a minimum of four years paid clinical experience developing, conducting, and approving assessments and Individual Service Plans of individuals served; or a Bachelor's degree in a human services field with one year of clinical experience highly preferred. Supervision available toward QMRP. A valid driver's license is required. Excellent training is provided. Starting salary of up to $35,186 (DOE). Salaried positions are eligible for benefits which include the Virginia Retirement System; life, medical, dental insurance; generous vacation, holiday, sick, and educational leave.
