Identify mentally ill homeless persons in the community, guide them towards and link them with available mental health and other community resources as well as provide training and consultation to area shelter staff. Assess, plan, link, monitor and advocate for clients to ensure appropriateness of services provided and continuity of care. Maintain Units of Service (UOS) expectations. Develop individualized service plans to meet client’s identified needs. Assist with liaison duties when requested and back up support to other case managers when requested. Maintain and update of service plan for all assigned clients (minimum quarterly reviews). Able to drive and use personal automobile.

REQUIREMENTS:

Bachelor’ degree in Psychology or related field or year for year related experience. Knowledge of and ability to link with available agency and community resources and support systems with working knowledge of homeless resources network. Ability to assess clients’mental status, evaluate the need for treatment and case management services, and advocate in the best interest of the client. Knowledge of crisis intervention, psychosocial development, psychopharmacology, and supportive counseling techniques. Ability to work under pressure and to respond appropriately in an emergency. Must possess valid Florida driver’s license and good driving record.
