Registered Nurse Case Manager: Girling Home Health Care

Ensures each assigned patient's Plan of Care for nursing and/or nursing and therapy cases is established, revised as indicated, appropriate to the patient, and in compliance with all applicable rules, regulations, programs, Agency policies, procedures, and processes. Responsible for the quality of patient care provided. Ensures that all aspects of patient care are timely and appropriately delivered. Coordinates interdisciplinary and interagency care as appropriate. Manages staff visiting assigned patients. Responsible for the clinical record for each assigned patient receiving nursing services and that all paperwork/reports are completed properly and timely. Responsible for customer service satisfaction. Provides direct patient care and on-call duties as required. Serves as a resource regarding patient care.

Qualifications

• Free from Medicare Sancations (required)
• Current RN license (required) preferably with a Bachelors degree in Nursing
• Current CPR certification (required)
• Adequate means of transportation to facilitate timely visits (required)
• Minimum 2 years experience as an RN (preferred)
• Knowledge of any type of Home Health software (preferred)
• Home Health experience or supervisory experience as a charge nurse, nursing supervisor, or in an ICU setting (preferred)
